Mid Norfolk Primary Care Network (PCN) is a local healthcare partnership that brings together neighbouring GP practices to work collaboratively, improving access to NHS services for patients in the Mid Norfolk area. Patients remain registered with their usual GP practice, but the PCN enables practices to coordinate more closely with one another.

Mid Norfolk Healthcare provides resources and services to support the PCN, helping to enhance access to care by pooling expertise across practices. Together with the GP practices, we work with local NHS partners, including community health providers, mental health services, and councils, to deliver more coordinated care across health and social care services.
